1. Assumption of Risk:
• I understand that art activities involve inherent risks, including but not limited to, potential for minor cuts, scrapes, splashes, hot glue burns, and exposure to art materials.
• I acknowledge that cARTizan studio and its instructors/staff are not responsible for any injuries, accidents, or damages to property that may occur during participation in art classes or activities.
• I voluntarily agree to assume all risks associated with my child’s participation in art classes and activities at cARTizan studio.
2. Safety and Conduct:
• I understand that my child will be expected to follow all instructions and safety guidelines provided by cARTizan studio staff.
• I will ensure that my child understands the importance of safe and respectful behavior within the studio.
• I understand that cARTizan studio reserves the right to remove my child from any class or activity if their behavior is deemed unsafe or consistently disruptive.
3. Medical Authorization:
• I hereby authorize cARTizan studio staff to administer first aid to my child in the event of a minor injury or illness.
• I understand that for any serious medical condition, I will be contacted, and I will be responsible for arranging transportation to a medical facility.
• I have provided all necessary medical information about my child, including any allergies or medical conditions, on the registration form.
4. Release of Liability:
• I hereby release and discharge cARTIzan studio, its owners, employees, and instructors from any and all claims, liabilities, or damages arising from my child’s participation in art classes and activities, except for those caused by their negligence.
5. Agreement:
• By checking the above box, I acknowledge that I have read and understand the terms and conditions of this waiver and agree to abide by them.
Privacy policy
Checkout form is to obtain your permission to use your child’s name, image, and/or information on our studio’s social media platforms and/or website (including but not limited to Facebook, Instagram, and the studio website) to showcase projects, activities, and studio events. We may use your child’s image without identifying factors to:
• Share photos and videos of studio events and activities.
• Highlight student projects and achievements.
• Communicate with parents and the wider community.
• Promote studio events and classes.
Opt-Out Option: You have the right to opt-out of having your child’s information shared on our social media platforms. Please indicate your choice below:
